OVERVIEW OF SERVICES

In all Innovative Services, Inc.'s programs, we:

  • Teach our clients skill sets to increase independence.

Vocational

Vocational programs at ISI provide rewarding and productive work with an incredible diversity of programs, including supported employment, enclave groups that work in the community and various workshop-based programs. Vocational work at ISI provides every individual with an opportunity to earn a fair wage regardless of his or her ability. Individuals learn social and work skills that can carry over to other aspects of life while activities and parties with co-workers and staff help build and improve social skills.

Adult Family Homes (AFH)

Adult Family Homes are state-licensed programs in which 4 or fewer non-related adults with disabilities live in a home in the community. Each individual receives care, treatment or services, above room and board and may receive up to seven (7) hours of nursing services per week. The smaller setting allows for more one-to-one attention and greater integration into the community. Innovative Services provides 24-hours per day, 7 days per week support to consumers living in AFH programs.

Community-Based Residential Facilities (CBRF)

Community-Based Residential Facilities (CBRFs) are state-licensed programs for 5 or more non-related adults, who are unable to live independently, but who do not require the services of a nursing home. Innovative Services provides CBRF services in a home in residential neighborhoods. Each individual receives assistance, as needed, for; personal grooming, meals, housekeeping, laundry, medication monitoring, up to three (3) hours of nursing care per week and has the opportunity to participate in leisure and social activities. Innovative Services provides 24-hours per day, 7 days per week support to consumers living in CBRF programs.

Diversion

The Innovative Services Diversion Facility is a medium Community Based Residential Facility (CBRF) serving up to 20 adults. The target group this program serves is adults with a primary diagnosis of mental illness who are in need of short-term crisis stabilization or short-term respite. Our main program goal is the fulfillment of our program philosophy. Innovative Services, Inc. is committed to flexible, accessible and family-centered supports and services that honor the dignity, respect the rights and maximize the potential of each individual receiving services. We are dedicated to providing the assistance we can to help our consumers achieve their goals and dreams, while fostering independence and maintaining self-respect. We believe the individuals we serve should be supported within the community in the least restrictive setting. We believe both our employees and those we serve have endless possibilities to be successful. Each person is unique, and the services required are diverse. The dreams and desires of the people we serve are the focal point of all we do, and we adapt our approaches and techniques accordingly. Innovative Services will provide each person with services consistent with their needs and abilities, while making them a part of a comfortable living situation. The clients of this home are referred through Brown County Community Treatment Program of the Human Services Department or the Family Services' Crisis Center. The program provides a coordinated series of services, including residential, community, leisure/recreation, and health monitoring services.

The facility provides for the basic services and needs of each client. All of these services will be provided with the overarching goal of recovery. Recovery is defined as a personal process of overcoming the negative impact of a psychiatric disability despite its continued presence. Staff will seek to enhance the self-direction of each client to allow them to take advantage of the following ongoing services that are provided. Supervision is provided to each of the clients at a level needed to insure safety and enhance community integration. Clients can be involved in a program of daily activities designed to meet the individual interests of the client. The types of activities will vary according to client preference. Community based activities will be utilized whenever possible. Clients will be encouraged to maintain healthy contact with family members and friends, as well as with case managers and medical professionals involved in the treatment of the individual. Medication education will be provided to ensure clients are knowledgeable about their specific medications and individual regimens. Each of these services is provided to maintain and enhance each individual's physical, mental, and emotional health while encouraging them to develop the skills and relationships needed to establish themselves and be successful in the community.

Supportive Home Care

Supportive Home Care provides services to persons living in their own homes and may consist of assistance with any of the following: personal grooming, medications, housekeeping, meal preparation, laundry, shopping, companionship, transportation and accompaniment for medical/social appointments. Clients select their own vendors, pay their own bills and purchase their own supplies. While client need dictates the number of weekly scheduled support hours, staff may be available 24 hours a day, 7 hours a day, to assist if this service is desired.

Supportive Apartment Program

Innovative Services' Supportive Apartment Programs are small communities of 3 to 30 individuals who rent their own apartments close to one another in an apartment complex. This program provides 24-hour staff availability to provide assistance, as needed, with any of the following: personal grooming, medications, housekeeping, meal preparation, laundry, shopping, companionship, leisure activities inclusive of transportation and accompaniment for medical/social appointments. By maintaining an office on site and through the use of assistive monitoring technology, staff are able to meet the needs of the clients in the complex.

Supplemental Support Services

Supplemental Support Services provides on-call support to consumers in need as well as positive behavior planning for caregivers serving individuals with challenging behaviors. Service is provided to consumers, with a broad spectrum of challenges, being served by Innovative Services, other corporate providers or in a traditional family home. The Supplemental Support Services Team in Brown County, Wisconsin, is comprised of highly trained program managers and staff, including two certified Non-Violent Crisis Intervention course instructors, who have worked with and are familiar with many Brown County consumers. On-call support responds to any situation that may cause an unsafe environment for the consumer or for others around them. Team members provide assistance in managing the situation on-site but may temporarily relocate individuals to ensure the wellbeing of all involved.

Comprehensive behavior support planning and training decreases the need for crisis response. Behavior support planning is designed to positively assist consumers and providers, ensures a person-centered service plan maximizes consumer potential and targets minimizing negative consumer behaviors. Supplemental Support Services were created to better serve consumers and providers and to reduce the need for crisis response.

Youth and Family Services

Innovative Services provides a variety of services for youth and families to promote personal growth that has a lasting impact. ISI provides support with life skills training, job coaching, relationship building, peer relationships, community service, daily living skills, social skills, recreational activities, experiential activities, parenting skills and 24-hour respite. These services are offered individually or in a group setting.

We create a unique learning experience for youth by challenging and supporting them in a natural environment. Youth learn effective problem-solving skills to help them deal with life's "real world" obstacles.
